Nursing Home/Hospital Transition

Assist people, age 60 and over, returning home after recuperation at a health care center or hospital.

Nursing Home/Hospital Transition VINE Faith in Action staff will meet with a care receiver prior to discharge from the health care center to develop a plan of care. Services are coordinated with the health care team, family care receiver and VINE volunteers. Our volunteers will provide assistance with non-professional services for up to four weeks after a care receiver returns home.

Volunteer services may include:

  1. Transportation to the doctor or grocery store
  2. Household and yard chores
  3. Running errands
  4. Meal preparations
  5. Reassurance phone calls
  6. Friendly visiting
  7. Durable Medical Equipment lending closet
Volunteers will generally be available Monday through Saturday.

There is no formal charge for this program, but we ask those who receive help to be as generous as possible with donations to our organization.
